₹5 lakh a year of cashless treatment per family, for families left out of the central Ayushman scheme
Uttar Pradesh runs its own health cover scheme for families that are not covered by the central Ayushman Bharat scheme. It gives the same kind of protection, up to ₹5 lakh per family per year, and treatment is cashless at hospitals that are part of the scheme. So a family that could not find its name in the central list still has a way to get hospital treatment paid for.
Who can apply
- Resident family of Uttar Pradesh
- Family not already covered under the central Ayushman Bharat PM-JAY list
- Family falls in a category the state has notified as eligible
Who cannot apply
- Families already covered under the central Ayushman Bharat PM-JAY
What to take with you
- Aadhaar card
- Ration card or family register extract
- Residence proof of Uttar Pradesh
How to apply
Get your card made at ayushmanup.in, at a Common Service Centre, or at the Ayushman counter of an empanelled government hospital in Baghpat district.

Most applications fail for a correctable reason, not because the applicant was ineligible. Before assuming you do not qualify, check each of these:
- Family is already in the central PM-JAY list, so a state card is not issued
- Family register or ration card details do not match the Aadhaar
